What is tesamorelin?

Tesamorelin (development code TH9507) was developed by Theratechnologies in Montreal. It keeps all 44 amino acids of human GHRH, the hypothalamic hormone that tells the pituitary to release growth hormone, and adds a six-carbon trans-3-hexenoyl group to the N-terminal tyrosine. Native GHRH is cut within minutes by the enzyme dipeptidyl peptidase-IV at exactly that end; the hexenoyl group makes tesamorelin resistant to it, and in plasma it breaks down more slowly than the native hormone.

Like GHRH, it acts on the GHRH receptor of pituitary somatotroph cells. The FDA label describes it as binding and stimulating human GHRH receptors in vitro with a potency similar to the endogenous hormone. That sets it apart from ipamorelin, which works through a different receptor (the ghrelin receptor), and from the shorter GHRH(1-29) analogues such as sermorelin and CJC-1295. See tesamorelin vs sermorelin and tesamorelin vs ipamorelin.

Certificates for the current lots

Each size is filled from its own lot and analysed separately by ILS Laboratories in San Diego, which states ISO/IEC 17025 accreditation for activities within its published scope. Identity is confirmed by HPLC retention time against a tesamorelin reference standard, and the certificate reports net peptide content in milligrams. The same ILS full QC panel also reports endotoxin, sterility, heavy metals by ICP-MS and a fentanyl screen:

Published research on tesamorelin

Tesamorelin is one of the few research peptides with a complete approval file behind it, so most of its literature is clinical. Each link goes to the PubMed record.

In people, the label gives a mean elimination half-life of 8 minutes (Egrifta SV) and 11 minutes (Egrifta WR) after a single subcutaneous dose. In dogs the apparent half-life was 21–45 minutes.

Regulatory status

Tesamorelin is an FDA-approved drug. FDA first approved it on 10 November 2010 as Egrifta (Theratechnologies), and the current US labels, Egrifta SV and Egrifta WR, are indicated for the reduction of excess abdominal fat in adults with HIV and lipodystrophy. The label says it is not indicated for weight-loss management and that its long-term cardiovascular safety has not been established. Egrifta SV prescribing information, DailyMed. Clean Peptide’s tesamorelin is a laboratory reference material, not Theratechnologies’ product, and is not for human or veterinary use.

QuestionsTESAMORELIN: frequently asked

What is tesamorelin?+

Tesamorelin (TH9507) is a synthetic analogue of growth hormone-releasing hormone: all 44 residues of human GHRH with a trans-3-hexenoyl group on the N-terminal tyrosine, which makes it resistant to dipeptidyl peptidase-IV. It acts on the GHRH receptor of the pituitary.

Is tesamorelin FDA-approved?+

Yes. FDA approved it in November 2010 as Egrifta (Theratechnologies). The current labels, Egrifta SV and Egrifta WR, cover one indication: reduction of excess abdominal fat in adults with HIV and lipodystrophy. Clean Peptide’s tesamorelin is a laboratory reference material, not that product.

What is the tesamorelin molecular weight?+

5135.9 Da as the free base (C221H366N72O67S), according to the FDA label. The CAS number is 218949-48-5; the acetate salt is 901758-09-6.

What is tesamorelin acetate?+

The acetate salt of tesamorelin, the form in the approved Egrifta product, whose label gives the formula C221H366N72O67S·xC2H4O2 (x about 7). Its CAS number is 901758-09-6; the free base is 218949-48-5. Research catalogues often list tesamorelin under this name.

How is tesamorelin different from sermorelin?+

Both act on the GHRH receptor. Sermorelin is the first 29 residues of GHRH with a C-terminal amide; tesamorelin is the full 44-residue sequence with an added hexenoyl group that protects its N-terminus from DPP-IV.

Is tesamorelin the same as ipamorelin?+

No. Tesamorelin is a 44-residue GHRH analogue acting on the GHRH receptor. Ipamorelin is a 5-residue peptide acting on the ghrelin receptor (GHS-R1a), a separate pathway for growth hormone release.

What does the tesamorelin certificate show?+

Each size has its own ILS Laboratories certificate, listed on this page with lot number, test date, HPLC purity, net peptide content and identity by HPLC retention time against a reference standard.
